Student Referral

Teachers refer students to the STARS Specialists using the STARS student assistance referral form. Teachers provide vital information regarding observable student behaviors in the classrooms: academic performance, classroom conduct, school attendance and behavior changes. Teachers also provide the STARS Specialists with actions they have already taken with the student, their relationship with the student and if they would like the student to know they made the referral. After the referred student is seen, teachers receive a STARS Action Notification form indicating an intake was provided and any continuing support the student will receive, such as individual and/or support group counseling, parent conference/contact or outside referral to a community resource.
